Self-Certification Program
Program Enrollment
Companies that are members of the Nashville Area Chamber of Commerce may enroll in the program for an annual fee of $250. The price is valid for one year or any part thereof and covers an official seal press and a rubber hold-harmless stamp. The equipment is used to certify that Certificates of Origin are correct to the best of the participant's knowledge and belief that the products are manufactured in the United States.
Length and Renewal of Term
The seal will be leased to the participant in January each year and will be valid for one year or any part thereof, beginning at time of enrollment. Participation is renewable annually. The Chamber reserves the right to refuse renewal of a participant's lease should a participant be deemed to be in violation of the program regulations.
Copies of Certificates of Origin
There is no limit to the number of documents certified during the year. Participants are required each
month to send copies of certified Certificates of Origins to the Chamber for review. The Chamber is responsible in turn for forwarding copies to the Department of Commerce. Participants must mail copies to the Chamber within the first week of the month for the previous month. Failure to mail copies to the Chamber on time may lead to termination of the participant from the program.

Annual Renewal
The Chamber will issue participants with renewal invoices each January. Participants must submit full
payment ($250) before receiving a current seal. Pro-rating of payment is not permitted. Checks should
be made payable to The Nashville Area Chamber of Commerce and mailed to:

Exclusive Use for Certificate of Origins
The embossed seal and hold-harmless stamp provided by the Chamber to participants are to be used exclusively by the participants and are for the certification of Certificates of Origins as may be required by the Department of Commerce for exporting. Any use other than for these purposes is strictly prohibited. The stamp and seal may be used by any of the participant's employees.
Accuracy of Certificates of Origin
The Chamber will contact the Department of Commerce, the receiving party and the participant in the event of inaccuracies on a self-certified Certificate of Origin. Repeated errors may result in an audit of the participant and dismissal of the participant from the program.
Participation Discontinuation
Should a participant wish to discontinue the Self-Certification Program the Chamber's seal and stamp must be immediately returned to the Chamber. No refunds will be made.
