Position Description / Responsibilities
Full-time Bachelor level position to engage new or existing Centerstone clients prior to discharge from a psychiatric hospital in the Davidson/Williamson county area in order to provide improved coordination of care between the client, the discharging hospital, and the Centerstone clinic providing outpatient care.
Applicants need to be willing to work within a team structure, possess an engaging and non-confrontational communication style, be flexible and adaptable to change, and possess critical-thinking as well as problem-solving skills.
Experience in dealing with clients with suicidal ideations or attempts, mental health concerns, and A&D concerns is preferred. Basic knowledge of Microsoft Excel is preferred but not required. No weekends, holidays, or on-call hours are required.
Job tasks include:
Meeting with the client prior to discharge from the psychiatric hospital in order to provide appointment information to the client as well as to discuss potential barriers that may hinder the client from attending their initial hospital discharge appointment at Centerstone.
Contacting the client one day prior to their initial hospital discharge appointment in order to remind the client about their appointment.
Following up with clients who missed their initial appointment to determine if the client needs to reschedule the missed or cancelled appointment.
Assisting Centerstone clinics in obtaining discharge paperwork from the psychiatric hospital, as needed.
Fielding any questions or concerns from a Centerstone clinic to the psychiatric hospital or from the psychiatric hospital to a Centerstone clinic.
Maintain a tracking sheet in order to document client information to include number of initial appointments kept or missed, number of referrals made by the hospital each month, and client demographics.
Bachelor’s level degree in Counseling, Psychology, Social Work or related field and at least one year post-Bachelor experience working in a mental health setting preferably relating to the fields of suicide prevention and/or alcohol and drug treatment.
