Position Description / Responsibilities

Café Momentum Nashville seeks an experienced Case Manager to assess, plan, implement, monitor, and evaluate actions required to meet interns’ health and human service needs. As a part of the Program Team, this person works closely with the Program Manager and Culinary Team to support programming and catering operations, ensuring interns are properly trained for service. Case Managers report to the Program Manager. They work collaboratively with other team members within the program to ensure the needs of the interns are met by taking a holistic approach. Case Managers also are responsible for cultivating and maintaining relationships with community organizations that provide services and support for interns.

Job Responsibilities
Assist with facilitating new intern orientation and the completion of all required documentation (enrollment forms, assessment, waivers/releases, etc.)
Provides comprehensive case management services; completes needs assessments, makes referrals and coordinating services, provides support, and promotes self-advocacy.
Maintain lines of communication with parents/guardians and other service professionals including but not limited to probation officers, case workers, school counselors, legal representatives, and mental health professionals.
Supports outreach and recruitment efforts by coordinating contact and events with referral partners.
Maintains up-to-date case notes on assigned interns.
Works closely with the Program Manager and Culinary instructor to ensure consistent support and alignment with intern strategies and delivery
Regularly meets with Program Manager at weekly 1:1 supervision
Communicates interns’ progress by facilitating regular meetings and disseminating results and obstacles providing guidance and instruction to colleagues on youth behavioral plans.
Assists with intern scheduling by communicating dates and times.
Assists with facilitation of life and social skills lessons; including but not limited to coordinating enrichment activities and community service opportunities.
Manages caseload by verifying interns’ attendance, observing and evaluating interventions and responses, advocating for needed services and entitlements, obtaining additional resources, and intervening in crises
Performs other duties as assigned

Position Requirements

Bachelor’s degree from an accredited college or university in Education, Behavioral Science, Counseling, Social Work, Psychology, or Sociology, or in a job-related field of study with two to five years of work-related experience
Case management experience preferred
Experience with trauma-informed care and positive youth development
Demonstrates an ability to produce a high volume of work that is complete, accurate, and of high quality in a timely manner
Demonstrates an ability to interact positively with supervisors, management, coworkers, and guests to promote a team effort and maintain a positive professional approach.
Must be a proactive self-starter and take initiative to identify and complete tasks to assist team members to work effectively and efficiently.
Demonstrates an ability to lead, inspire, and support a team towards an impactful program and team experience.
Ability to work well in a team environment and willingness to exceed expectations is required to be successful in this position.
Positive disposition, attention to detail, flexibility, and adaptability with the ability to learn new tasks quickly while multi-tasking many responsibilities
Experience working with youth from various cultural backgrounds
Passion for working with youth as evidenced by past experiences and ability to build relationships
Good role model to young people in personal character, integrity, and behavior.
Creative and energetic
Google Suite proficient
Must submit to and pass a background check prior to final employment offer
Must be cleared to drive interns and insurable on catering vehicles
